Random CTDs In Games - Same Exception Code C0000005
May 9, 2014
what I am experiencing are totaly random CTDs of any game I play, it has been happening In Win 7 and now with Win 8.1. All same kind of crash with same exception code.
Games have been Battlefield 4, Hitman Absolution,Left 4 Dead 2 and now Red Orchestra 2.
Here is what reliability monitors report has to say about the latest crash:

Notice the exception code, c0000005,every single CTD with every game was giving this same exception code....
At first I suspected a hardware problem since that code usually means defective RAM. Thing is I have tried everything I could think of. System is not overclocked, ran memtest on each stick individually with no errors, swapped RAM with memory from a functioning computer,RMAed mobo, even just swapped video, and still same problem. Only 2 parts I havent touched are CPU and PSU. I suspect maybe a defective memory controller on this CPU? Also to rule out software issue I tried running driver verifier, but when I restart it doesnt seem to be doing anything.
I've recently upgraded my motherboard, processor and RAM to an ASUS Z87-K, Intel i5-4670K, and Corsair XMS3 1x8GB, and ever since I have been experiencing what appear to be video card related crashes. I am using a Zotac Geforce GTX 560 AMP, and a Corsair 750w PSU and did not have issues before the upgrade. The computer will crash to desktop while playing high performance games, sometimes almost immediately, sometimes after an hour or two. This results in an event log message like this:
This is bad enough, but I have also been receiving BSOD in more sever cases. My dump files are zipped and attached here.
In my troubleshooting to date I have: - started upgrade from clean Windows 8.1 install - checked all my power supply settings, - checked all internal connections - made sure the RAM was running at the appropriate frequency and power settings - updated my BIOS and all my drivers manually, included several clean driver installs for my graphics card - conducted Windows Repair - checked the temperature: CPU less than 40C, GPU a fair bit higher in mid 70's - tested memory for damage, came back clean - ran dskchk, came back clean - ran dxdiag, says there are no problems
Note: I've been using Shogun 2 to run tests, but it happens on every game I've tried (Arma 3 and Planetside 2 through Steam, and FarCry 3 through Uplay)
My next step will most likely be another complete format and reinstall, and then if it still remains I would have to assume my video card is shot, a horrible coincidence that it happened right when I upgraded everything else.
My pc just started crashing a few days ago. I will try to be brief because guess what you all really need to see is the zip file. I have tried a few things to work with this code 41 kernel and I am not sure what is causing it. I am mostly crashing it seems when I try to use windows features/tools. It cant seem to complete any recovery etc tools before crashing. I have attached this file...
I'm having this BSOD since months ago and I can't find a good solution. Sometimes I can use my computer for over a month with no issues playing everyday for hours.. browsing the web etc. But sometimes like today I get 2 BSOD's on the same day playing for example Final Fantasy XIV ARR.
I tried many things before like Memtest, Cpu test, etc and I can't understand if my Hardware is the problem or some driver. I also have an after market Cpu Cooler so my temps are fine and my case has really good ventilation.
I uploaded this dumps a few days ago after I used Driver Veryfier. I got a C4 BC Code error on those Minidumps and the problem was nroskrnl.exe. Only with Driver Veryfier I'm able to reproduce this BSOD.. is always random when I'm just using my computer.
I uploaded those dumps here [URL] .....
I uploaded my latest dumps on this post using the SF Diagnostics Tool..
I have a problem with my machine I've been battling for months now. It will just randomly restart and I could be doing anything. It has happened when browsing the web, playing games, and even when the motherboard logo shows up during startup.
It never seems to crash when running Prime95 or OCCT stress tests. What I've done:
Hardware -Re-installed windows twice -Purchased new motherboard -Purchased new CPU cooler (from air into liquid) -Purchased new GPU (HD7950 to GTX780) -Purchased SSD -Monitoring software showed PSU 12v rail running at 10.8v when stressed. Checked 4 pin molex connector with voltmeter and it measured 12.2v and it dropped to 12.1v under stress. -Changed the case, thought it was a short circuit somewhere.
Software -Run multiple stress tests which did not cause a crash. -Ran Prime95 and Battlefield 4 at the same time. No crash -Updated GPU drivers -Updated motherboard drivers
-Can't update BIOS, scared computer will restart during the flash. My motherboard revision is 1.01 and -I've run memtest for 5 hours, but I should do it again
Sometimes, instead of a restart, my display will turn off and any sound playing will begin to loop. PC will be unresponsive. CPU temps reach only about 35 degrees under full load.
So, my computer has been getting the BSOD lately for the past month or so. I get at least one BSOD everyday and sometimes two or three of them. From 12 am to now, I have gotten the BSOD three times. It happens when I play games too, but I don't think the games are the cause of the BSODs because they occur even when I'm not playing. I play CS: GO and Borderlands 2 if that will work. The BSODs are always IRQL_NOT_LESS_OR_EQUAL errors and then BlueScreenView says it has something to do with ntoskrnl.exe. I have attached the ZIP file that the SF Diagnostic Tool recommends.
My problem is only happening when gaming and at random times, i also have crashes to desktop, but i dont know if the 2 problems are realated. Anyways lately i have been getting some BSODS thats mainly looking to be related to ntoskrnl.exe and ntkrnlmp.exe when i look in "whocrashed"
I uploaded the .zip from the SFD program from your website along with the latest minidump files (080514-4031-01 beeing the latest).
It only occurred first if and only if I was playing, so I thought it was a compatibility prob so I just quit. But when I played this game Dragon Nest it too BSOD's or just plain PC crash. So I thought ok I cant play these 2 games. BUT when it happened too in Dota2, I think that's too much of a coincidence. Now my PC just random crashes and I really dont know why.
PC specs: Core i3-3240 Geforce GT420 4g ram Windows 8 64bit
Ive been having some random BSOD`s all pointing to different files, i`ve even had two today (of which i can only give one error code since the other BSOD was all teared up and unreadable) I`ve had one that said:
Driver_IRQL_Not_Less_Or_Equal afd.sy
The other BSOD`s were not legible. Im currently using windows 8 and not 8.1 since i am a gamer and i`ve heard a lot of bad things about 8.1 and gaming so i never upgraded. I`m at a loss of what to do here on a side note i`ve also noticed that some games (usually steam games and particularly a game called Dust: an Elysian tail) keep crashing with the notification:
I just got an Asus Q501LA notebook and while connecting to a wireless network I get a BSOD with this message kmode exception not handled (netwew02.sys)
All of them result in a file called ntoskrnl.exe being highlighted in BlueScreenView Other files highlighted in KMODE Dump were athw8x.sys, Ndu.sys, and NETIO.sys
I've been getting blue screens of death infrequently, attached to my PC randomly losing power and hard freezes. I used to ignore it (used to get it rarely in BF4 a while ago, and often in Star-Citizen), but with things like LoL or Diablo 3, it doesn't ever happen.
Now i've been playing ArcheAge it's been happening alot, I purged my ATI drivers (was getting an AODDriver registry issue showing up with Event Viewer) with uninstall and the reg-edit clean. Then installed the most recent build and used CC cleaner to clean out a whole lot of issues it found. The random shutdowns and freezes seemed to have stopped, but I've still been getting the System_Service_Exception BSOD a few times a night of constant play.
I have a bad suspicion with the freezes / shutdowns it's a HDD issue or a power supply issue, but after the driver clean i've only been getting BSODs so far (as the oldest parts of my PC is my 5+ year old Corsair 620W and my HDDs that now are sitting loose as my 280x is too wide for the case(getting a 750D and a SSD for boot soon hopefully, then PSU later).
Still getting freezes. I think the attached ZIP is what i need to attach, I accidentally ran it twice not realising the folder where it put the dump.
Internet Exception Error 12003 Details: The server returned extended information 226 Transfer complete. 200 Type set to I. Just started after updating to 8.1
I have a program called WYSIWYG Web Builder. It has FTP built into the program. It uses wininet. I have had no problems with this program until now. It was working fine with Windows 8. After updating to Windows 8.1 I now get this message.
System Thread Exception Not Handled AND Failure Configuration Windows Updates, Reverting Changes.
I own a computer Asus (Windows 8, 8 Gb RAM, 1 Tb hard disk, Intel i5 cpu). I cannot use that computer, because every time I have to restart the system, I get a Blue Screen Of Death: the message says "System Thread Exception Not Handled". Restore-points have not solved the problem. I was forced to completely reinstall the OS. Over and over again. The last time it happened, I was forced to Reinstall the OS twice. I was awake for nearly 48 hours, non-stop, to make sure everything would go smoothly.
Every time I reinstall the OS, I am forced to reinstall all the hundreds of Windows Updates. On a Metered connection. In one week I've used up my ISP's monthly internet allowance.
But this is only the beginning of the nightmare. Not only do I have to re-download and re-install hundreds of Windows Updates, over and over again, and on a Metered connection. But, also, every time I reinstall them, I get the message "Failure configuring Windows Updates; Reverting Changes." What is ever worse is that the message only appears AFTER I've downloaded gigabytes of Windows Updates on a Metered connection.
In other words, Microsoft Windows asks me to install Updates, to solve the problem "System Thread Exception Not Handled." But, at the same time, Windows is incapable of installing those Updates, because of "Failure to configuring Windows Updates; Reverting Changes."
So, in order to be able to use that computer, I would like to completely remove Windows from that computer. But the computer does not allow me to format the C drive. It does not allow me to delete Windows 8, either. What should I do??
My Msi Ge70 2oe is 5 moths old and i got several BSODs since purchase. I updated most drivers in April, still got 6xBSOD this month. Most of them System_Service_Exception and addressing ntoskrnl.exe+59769. idk what that means, think have some really bad drivers it happens mostly using chrome.
Using manufacturers Live update 5 to update, although not installed some optional like norton internet security. Checked memtest and windows memory test programs, nothing unusual.
I used to blue screen whenever I start up my laptop; could only start it through safe mode. I had a ton of errors trying to reinstall Windows, but then eventually did it. Anyway, Why I was blue screening. I was setting up everything and downloading things I wanted (after resetting Windows and deleting almost everything), when *SEEMINGLY* out of nowhere, I would blue screen. After a couple of blue screens, I noticed it was only when opening Skype. Turns out this was why I was blue screening ages ago when opening my laptop. Anyway, I want to fix this. More detail would be this: I click on Skype, blue screen with something to do with "System service exception ks.sys"
I need Skype to communicate with friends and family.
I am just built a new pc and installed and updated all drivers properly. But it keeps crushing and most common bsod is "Ntfs_File_System" and "Service_Exception_Not_Handled". I searched almost all of similar forum post and i still get bsod's. İ have tried to run Memtest86 and it couldnt find any error in rams. Sometimes data corruption occured in my several files and i have ran HD TUNE to find any bad sector but it was ok too. I have done all i can and it still crushes 2-3 times a day.

ALT-TAB is not working when I am playing online games (Crossfire -develop by SmileGate/Neowiz) and Steam-DOTA2. I just experience it when I upgrade from Windows 8 to Windows 8.1. Is there a workaround on this? I don't want to go back to Windows 7 or 8
I just updated my Nvidea graphics card, and for some reason after restarting my laptop, in order to install these files to the system, the "System Thread Exception Not Handled" Blue screen error flashed on my screen, and later redirected me to an automatic repair screen, which attempted to fix this issue.
I ended up restarting the laptop on safe mode, uninstall my Nvidea graphics card and completely reinstall it from scratch. But for some reason it keeps giving me the same error each time I restart my laptop. After some research online, apparently there's an issue with Avast and Nvidea?
You will find attached the SF Diagnostic Tool grab all option files needed to view the issue at hand. Many are saying that I've not been installing the corrent drivers, but that's not possible, as it's directly from the MSI website (GT70 0NE) linked to the official Nvidea website, saying that the driver includes my graphics card (680M).
While other people are saying that maybe Avast is causing some issue towards the graphics card?I have the Avast 2014 Professional Antivirus.

I'm currently running my laptop without the nVidea Graphics Card Drivers, and seems to be fine so far.
I have a Surface tablet. When trying to download an app I get sent to the security verification page where you eventually are emailed a code that you must enter to continue. How do I get back to that page after checking my email? If I start over again downloading the app I wind up back at the start of the verification process and the code I was sent no longer works. At home I can use my phone to get the email, but out of the country I may not be able to use my phone.
i have recently bought a new laptop, windows 8. Its a Lenovo Ideapad Y50.
At first i had a password and a pincode, i was able to switch between the 2 login options at the login screen, i always used the pincode, as it was faster. I forgot the password. And somehow the option to choose between the 2, is now gone. To conclude, i am unablebto login to my laptop, as i dont know the password and it is not allowing me to login through my pincode. The account is not a microsoft account, so i cant just go to there website and change my password. Nor do i have a password reset disk. I have tried all the passwords i know and have no clue what to do next.
When I do a search of the registry, I get the following errors:
Failed to open HKEY_LOCAL_MACHINESOFTWAREClassesWow6432Nodeigfxdv32.CUIDriver while searching the Registry on Local Computer, Windows reported error code 2.
Registry Search Error. Failed to open HKEY_LOCAL_MACHINESOFTWAREClassesWow6432Nodeigfxdv32.CUIDriver.1 while searching the Registry on Local Computer, Windows reported error code 2.

If I try to delete or modify any of these entries, they cannot be accessed through regedit but are available to PsExec.exe from System Internals. I haven't tried deleting these entries for obvious reasons since I do not know if they are essential or not.
I don't use metro, but there is this one app I really like. So I want to buy it. When I try this, I have to fill in my Email adress which for me is the preferred way to receive it. I follow up all the instructions, fill in the right adresses and wait.....But I never get a code. I've got a MS account.
I recently built a new pc and installed 8.1. But i've been getting infrequent bdsod's with the code System_Service_Exception. I've installed all of the latest drivers as part of the new build with the only exception being the drivers for my Plantronic Gamecom 780 headphones with Plantronics unable to produce a windows 8 driver for them.
Windows 8 factory install on HP 500-056 updated to Windows 8.1 from the Microsoft Store.
Since the update, when I shut my computer down at night, there's a mysterious error code that doesn't stay on the desktop long enough for me to read it.
It doesn't seem to be causing any problems, but it's an error code and I don't want it there, needless to say.
I've just got Windows 8, and I'm really happy for it. My only problem is that I accident wrote my activation code wrong in the P c-Settings, and now i am not able to change it. It just keeps coming with this error. How am i able to change it?
I have bought lenovo g50-70 just 20days back, it came with inbuilt windows 8.1, they did not give any recoverable disks with it rather they said to take back up of the OS into a usb stick but I forgot to take the backup and tried to partition the hard disk using EASEUS Partition manager then I got this error "0XC0000225"
