Debugging :: Random Freezes ( Black Screens )
Sep 15, 2014
I'm having problems with my newly built computer, i build this badboy 3 days ago but it has lots and lots of problems :/. Whenever i play a video game it crashes after around 10 minutes of gameplay, i can hear the speakers buzzing when it crashes and it won't throw me any error ( nor am i getting a BSOD ). I checked the event logs but my knowledge has failed me!
Please see the attachment for the LOG files.

I'm getting either the KERNEL_SECURITY_CHECK_FAILURE or the BAD_POOL_HEADER Blue Screen (of Death).
According to BlueScreenView, ntoskrnl.exe is responsible for most of the BSODs. You will find the dump files and a list of all the hardware specs of my computer in the attached rar archive.
My OS is Windows 8 Professional (x64).

Mini dump files.
Computer specs:
Motherboard: Gigabyte z77x-ud3h
i7 3770k, never overclocked running stock
16GB crucial ballistix 4x4GB
EVGA Nvidia Geforce GTX570 HD
Antec high current 750w gamer PSU
Problem: My computer crashes randomly, sometimes while I'm working, sometimes while I'm away from the computer, there doesn't seem to be a pattern. Oddly enough, windows 8.0 ran just fine and never crashed. Windows 8.1 seems to crash much more frequently, and either blue screen or just lock up and reboot with no blue screen.
Suspected potential problem: When upgrading to windows 8.1, in order to get my very old cisco VPN client working, I had to install a citrix DNE (deterministic network) driver. I'm guessing it could be related to this and the old cisco client, though I have unchecked DNE on the network adapter and I have still had crashes, even with it unchecked/disabled in theory.

Not sure what is going on right now. My computer was fine this morning before I left for work. My wife said it was working fine this morning, and then she left it for a while and when it came back, she was getting blank screens.
When I took a look, it was blank. I did a hard shutdown and after the welcome screen, got a black screen of death. After several reboots, it finally got to the tile screen, but froze there.
So, right now, I'm operating out of safe mode where things seem more normal. Anyway, here are the logs.

Basically every app i open, every little things i do, would end up being unresponsive for up to a minute, then it would work for a few seconds and freeze again. I cant surf the web, play games, or even go to the start menu without everything freezing on me. Heck, typing a 3 letter word would take an eternity.
I've done a ton of research and did everything i can think of to fix this issue. I've refreshed my windows not once, or twice, but THREE times and this problem persists. After the refresh everything would go smoothly for a day then everything gets progressively unreponsive. I've also booted in safe mode and ran both malwarebytes and superantispyware. The scans ended with a few threats and ive quarantined and deleted every malware.
Next thing i did was doing the sfc /scannow in command prompt. It detected some corrupted files and successfuly fixed it yet the problem still persists. My last hope was upgrading to windows 8.1 but alas, but even that didnt do the job. Everything is not responding and its driving me nuts.
I'm typing this under safe mode with networking (everything seems to be completely smooth and responsive in safe mode).

I've had this Dell XPS for the last 3 years I guess and over the past 4 or 5 months, my computer has been randomly restarting, freezing and now today crashing. Sometimes I'll be on it and it will restart, sometimes I won't even be near it when it restarts. Last week I was on it and my screen went half blue, but I could still see part of my browser but it was frozen. So I just had to cut power. Here is a picture of said incident. [URL] ....
I've also had it freeze and then I come in the room and my tower sounds like a jet engine and I have to turn it off. Then yesterday the blue (as seen above) cascaded down my screen, froze and gave me the BSOD. I've had 3 crashes in less then 24 hours and I'm at a loss as to what to do. I think I have every driver updated, but maybe I'm missing something.

I am having a slight issue with what I am assuming is my video card. I have a Powercolor HD7870 Tahiti MYST Edition and on some screens I will get these random rectangles that show up. I have a few pictures to show what I am talking about.
It almost always happens when I am on Twitter and most other websites dont have this issue. It is more annoying than anything else, it doesn't seem to affect anything but I figured that I would post here and hope that it isn't a big issue.

Since yesterday, my graphics card has been starting to go to black screens and emit loud fan noises. I suspect it may be due to a recent update regarding something about WDDM 1.3 since that too was installed yesterday.
I have managed to play The Sims 3 at max settings today without the problem, but the problem kept recurring after that. Even with non-intensive activities like watching YouTube brought my desktop to a screaming halt, literally.

So, I am having blue screens and shut off on my laptop with 8.1 and 8 before.
I tried everything. Clean installs, BIOS and all drivers update...
It gets blue, shuts off and restarts to DOS, and I have to remove the battery and unplug to restart to Windows.

Since last week I'm getting a lot of blue screens which appear for no apparent reason, sometimes I leave the computer locked for dinner and when I come back, the computer has restarted with a blue screen.
I'm using Windows 8.1 with update 1, and the blue screen's message is IRQL_NOT_LESS_OR_EQUAL .
I suspect that this is happening because of a wi-fi receiver that I've installed on my computer a month ago.

So I have been getting Blue screens on and off for a fair time now ( few months ) from once per week to multiple times per day. I have been trying to fix the problem for a fair while but I feel I am getting nowhere in which the frequency of the blue screens doesn't seem to change no matter.
Steps I have taken so far:
- cmd>sfc /scannow: This might have been a culprit to some blue screens. At first when run I would get "corrupt files found but not fixed". After spending some time trying to refresh windows 8.1 and failing (as I only had Windows 8 key), eventually I did a clean install of windows 8 (and still having the same message). I have since acquired a windows 8.1 key and used that to refresh windows, and now the message is from sfc when run is "Windows Resource Protection did not find any integrity violations".
- Windows memory test : So I ran this and had no errors.
- cmd>verifier: I have recently tried that with no crashes ( I think I set it up correctly and when I rebooted it booted normally to windows following steps here : [URL] ....).
- Moved graphics card slot: So I move my graphics card down one slot on my motherboard in case there was a dodgy connection, but this didn't work
I have attached the output from the SF Diagnostic tool and also an additional text file generated from WhoCrashed with some additional information.

I recently built my first custom PC. Here is a link to the build on PCPartPicker: AMD FX-6300, Sapphire Radeon R9 270, Corsair 300R - Ordered build updated - ajthemacboy's Saved Part List - PCPartPicker
Since then I have had random black screen issues. At one point I reinstalled the OS, and they still occur. You may notice the R9 270 needs a 450W power supply at the least.
At first look you would think this is the problem, but it's not. Before the R9 270 I had an EVGA 750 Ti, which only uses like 60 watts of power or something, as well as a CM430W power supply, and the black screen issues still occurred.
First I returned the power supply for a CS450M, black screen still occurred. Then I returned the 750Ti for an r9 270, and the issue still occurs.
If it's not the graphics card, power supply, or OS, what should I do?
The dump file is attached. It blames NTOSKRNL.EXE, but I have no clue what this is or how to fix it.

I bought a new laptop (Vaio Pro) a couple weeks ago, and it's been bluescreening repeatedly ever since (about 20 times in two weeks). I'd send it back to the manufacturer, but I'm currently being interviewed for several jobs, each of which requires an interview presentation, so I absolutely definitely need access to it at the moment, and that's unlikely to change in the next few weeks sadly.
I've attached the Diagnostic Tool output.
The error messages appear unrelated, which I would guess is symptomatic of a fairly low level problem.? The most common ones are "DRIVER_IRQL_NOT_LESS_OR_EQUAL" and "PAGE_FAULT_IN_NONPAGED_AREA", but there are others (see dump files).
There's no obvious pattern to when the crashes occur, except maybe a very slight increased probability when playing a game (although it still happens when I'm working as well) .

Ever since I switch to windows 8.1, I usually get random black bars in chrome when i switch to other apps. The black bars goes away when I move my mouse over it. This doesn't happen on windows 8 or windows 7. I have HD 6970 gpu and the latest video card driver.
Also, I notice that in windows 7 and win 8, when i right click the chrome icon on the task bar, it would have options like new window and new incognito window, but in window 8.1 it's gone.

I've recently assembled my pc and everything seemed to be working fine. But one day my pc was frozen on the window menu. I hard to forceably turn off my computer and restart it. When i turned it back down my pc would turn on and off repeatedly until it finally turned on for good. When my computer freezes, my keyboard, mouse and headset Immediately turns off, basically anything connected to the mobo. here are my specs
OS: window 8.1
MOBO: Asus P9x79 WS
CPU: intel i7 4930k 3.4ghz
GPU: 2x 780 ti's
Ram: Corsair Dominator 16gb 2400mhz (4x4gb)
PSU: Corsair AX1200i
Hardrive: 2tb Seagate SSHD
I've enabled XMP to run the ram at recommended settings, and disabled it, both options didn't solve the problem. It happens randomly, it could be on running fine for 8-10hours but out of no where the computer freezes acts wild (shuts itself off and on by itself).

A couple of weeks ago my old PSU blew up. It did kind of like a explosion sound and some flashing, very impressive. I inspected the motherboard and I could not find any damage whatsoever. Only the PSU seemed affected.
After that I bought a new PSU (Tacens MP700 - 700W), an aftermarket CPU fan (Cooler Master Hyper TX3 EVO) and some extra chasis fan to improve air flow and lower the temperature. I plugged everything in, and system started with no apparent issues.
However since a few days ago sometimes my PC restarts without saying anything, some other time it just freezes (very annoying) and some other times it gives me a BSOD with the errors CLOCK_WATCHDOG_TIMEOUT. The frequency of these failures:
- Normal use like browsing, email, programming --> not very often
- Playing game like iRacing --> sometimes it freezes
- Playing game like PES2014 --> it always freezes, sometimes during the first match, sometimes after a 2-3 of them
After googling it appears is a hardware problem so I wonder if the damn PSU damaged some of my other components. The thing is the original PSU was intended to be for a quiet HTPC but I started to work from home and suddenly became a work station. I should have improved the PSU among with the other components (CPU, memory, etc.) but I did not think it was necessary. Bad decission I guess.
I've monitored temperature and it does not seem to be the issue as using a benchmark software at 100% CPU during 30 minutes temperature does not go over 65C on the CPU (I live in Spain where ambient temperature is around 30) and motherboard never pass 68-70. Right now while browsing temp is 35 for the CPU, 45 for the mobo and graphic, 30 for HDD. Pretty normal I think.
I have also upgraded from Windows 8 to Windows 8.1 hoping it was a driver issue but not success either.
Find attached the SF report. Added new crash info:

I recently bought a Samsung EVO 840 120 gb SSD and did a clean install of Windows 8.1 Pro on it. Also formatted my other HDD's. And after that I started to experience at random times the complete freeze of windows and sometimes blue screen of death with "clock watchdog timeout" message. They usually happens when I unpacking an archive, during file copy, downloading big files, installing mods in nexus mod manager for skyrim or sometimes just in very random events. I noticed that usually when this happens hdd activity led on my pc case blinks continuously. When this freeze or BSOD happens I need to manually reset my computer.
(Also most of the archives I download from the internet I get crc errors for most when I try to unpack them, so I have redownload it again :/)
My system:
OS: Windows 8.1 Pro
CPU: AMD Bulldozer FX 8150 Eight core Black Edition
CPU Cooling: Hydro Series H60 High Performance Liquid CPU Cooler
Motherboard: GA-970A-D3 (Rev. 1.0)
Memory: hiper blue 8 GB DDR 3
Graphics Card: Gigabyte AMD HD 6970 2 GB
PSU: Gigabyte 650 W
Hard Drives:
-Samsung EVO 840 SSD 120 GB (OS)
-Samsung HDD 500 GB
-WD HDD 2 TB
Nothing is overclocked. Everything has the latest drivers. Bios is updated to the latest.
I did CPU stress test got no errors.
I did GPU stress test got no errors.
I did RAM test got no errors.

For the past couple of months, I have been noticing unexpected freezes on Windows 8. Since I have access to MSDN, I decided to do a fresh install of Windows 8.1 to see if it solves my problem.
Now, when it freezes, it actually shows me some useful information. BSOD is labled as BAD_POOL_HEADER and I have grabbed the 2 dump files and other info in the zip file attached to this post.
Memtest and Chkdsk show no problems after running both. Memtest did about 6 passes last night without any errors so I know for sure it's not a problem with my RAM. Chkdsk showed no errors after running it in my 8.1 bootable USB so my SSD is fine. I also have the latest Kingston SSD firmware. NVIDIA Graphics card drivers are also updated to support Windows 8.1. Still not sure what's causing it.
BlueScreenView shows the following confilcts:

Bought a new laptop almost a year ago, i was never able to find the cause of the crashes.
Here some specs
- Intel i7-3537U CPU
- Asus K56CB motherboard
- NVIDIA GeForce GT 740m Dedicated GPU
- Intel HD Graphics 4000 Integrated GPU
- 4gb 1600 Ram, single stick
- Windows 8.1 (had the same problem with win 8)
On every game i play i get random freezes, random crashes, random BSODs, some games do crash more frequently than others, I've tried older drivers, I've also formatted my pc but the problem is still there...I am not using any antivirus, windows defender is enough for me.
What gets on my nerves the most is that even on old games like quake live do crash and sometime it also gives me BSODs, the fps are constant and quite high but i randomly get freezes (about 0,5 - 1 second long)...
another example, im playing SMITE, i get 80-150 fps but the game crashes every 2-5 mins, this game rarely gives me BSODs, when im lucky the game wont crash for 20-30min, but its very rare. the gpu can reach 80-85C. With "crash" i mean black screen with a window saying "Game.exe has stopped working"
There are other odds things happening on this notebook, for example i cant access to the bios while booting, i can only access to it once i log to my windows account, another strange thing is when i left click any file sometime it takes 10-15 seconds to get the "open, open with, copy, paste, cut etc.. window opened". I've also noticed that sometime while gaming my disk usage get to 100% for an instant and when that happens i get a lag spike/freeze...
If no way to find the source of my problems ill build a desktop and than ill keep sending the notebook to warranty until they fix it . I've still got 1 year of warranty, I've also sent it once already but they only changed the hard disk, and it didn't solve the problem too ...

I bought a new custom built PC from EveTech around 2 months ago for $1900.00. I installed windows 8.1 on it, and have just been receiving ENDLESS BSODs. At first I found out that Windows 8.1 does have issues with BSOD, so I installed Windows 8, hoping this would have been the solution.
I started doing some investigations on how to read BSODs, and initially I thought it was because of outdated drivers, bad memory etc, because all the posts that I read said that this is MOSTLY the cause of BSODs. I installed every program under the sun to search my drivers, and all stated that everything is updated...
Going further in the investigations, I thought it was because of overclocked CPU and GPU hardware, but even after resetting it back to factory default, I am still having this issue.
The absolute worst part of this, is that the BSODs are never the same - they are constantly throwing a new .sys file stating that "this is the problem".

I had a windows 7 previously installed upgraded it to windows 8 a couple of months ago. Since then, I'm getting random BSODs with errors including:
DRIVER_POWER_STATE_FAILURE
DPC_WATCHDOG_VIOLATION
REFERENCE_BY_POINTER
IRQL_NOT_LESS_OR_EQUAL
Previously with win7, I had a Windows 8 dually installed. Back then, I used to get the first error only, very frequently, and only in Windows 8. I had ignored them because I had Windows 8 installed only for testing purposes, and I thought they would go away. But now, they just keep popping up, with the reference by pointer error popping up 4 damn times in a row. And may I also add just how slow my PC gets before a BSOD?
I've tried looking them up, but the solutions seem to involve the removal of different drivers. So I thought of just creating a new thread instead. I have a few games installed and I do overclock my pc sometimes, but the recent BSOD (irql) took place when I was watching a video on youtube. I am attaching the SF grab and a bluescreenview report (the 5th error in it is the dpc watchdog violation).
This might be completely irrelevant, but I'll mention it anyway. The previous Windows 8 is still installed. I tried deleting it from msconfig, and was successful, but the files are still there and it seems to be the 'system partition' right now. See my previous thread for more info.
Happened again. Uploading SF grab with bluescreenview report.
Happened a few more times. I'm trying to keep the logs updated. Another thing; my PC now seems to be very slow and sometimes when a crash occurs, all the online accounts I'm logged into (Facebook, Eight Forums, etc.) automatically logout. This was also the case in my earlier Windows 8 installation. And the latest crash appears to be of a different driver.

Recently I've experienced random BSOD's in Windows 8. Although my computer know-how is limited at best, I knew it was best to upgraded my pc in steps. The first week I installed a SSD (with a fresh install of Windows 8 on it) and ran without any problems. A week later I installed 16 gigs of (new) ram (with the SSD) and everything ran fine.
The problems started two weeks later. I haven't made any changes to my pc or driver upgrades since then. If I idle in windows everything works fine. The same is true when running Office or browsing the internet. BSOD's occur when:
- Playing games (modern or old), either after 5 minutes or after 3 hours (seems very random but frequent);
- When wathing a HD youtube movie (BSOD's when doing this seem very rare though);
